Consulting Agreement:
The Counseling Center at CELA agreement for services form is to be completed prior to the scheduled consulting session. If you need help or support completing the form, it can be completed during your first or initial session. Your consultant can provide assistance. Please contact us and schedule your consultation before submitting the Agreement form.
